研究背景 要实现构建小康和谐社会的战略目标，必须促进农村经济、社会的全面进步。加强对农村卫生工作、尤其是农村居民健康状况的研究，成为当前积极发展农村卫生事业、逐步建立健全农村社会保障制度的重要内容之一。 在我国农村地区，伤害在疾病谱、死因谱以及住院疾病谱中的顺位逐步上升，已经成为农村地区的常见、多发性疾病，严重威胁着农村居民的健康和生命，伤害已经成为重大公共卫生问题。 目前我国对伤害的研究主要集中在流行病学研究方面，且研究对象局限：针对农村居民各种伤害类型进行的全人群抽样调查较为少见；对农村居民伤害的社会经济学研究，以及伤害患者与其他人群的健康状况、卫生筹资和卫生服务利用公平性的比较研究尚属空白。基于上述背景，我们开展了《山东省伤害流行现状及干预对策的研究》，并获得山东省卫生厅科研资助。 研究目的 在对农村居民人群基本健康状况、伤害流行病学现状及其危险因素研究的基础上，分析伤害的经济负担及其影响因素，评价农村地区不同收入人群、伤害人群与其它患病人群间的卫生服务公平性及其公平程度，分析病伤对不同住院患者的后续影响程度，为制定可行的意外伤害预防、控制措施以及相关的卫生经济政策提供依据，以切实降低农村居民伤害的发生及其疾病经济负担，减少“因伤害致贫、因伤害返贫”现象的发生。 研究方法 采用分层随机抽样和机械抽样相结合的方法抽取样本家庭；对样本家庭的所有常住人口，利用调查表通过面对面访谈方法收集资料。共调查农村居民家庭4329户，调查人口13919人。使用FoxPro建立数据库，利用SPSS统计软件包进行数据处理和分析。 在对伤害发生情况进行描述性研究和分析性研究的基础上，利用Logistic回归分析方法探讨伤害发生的影响因素；利用研究者改良的人力资本法测算伤害的
BackgroundThe improvement of social-economic status is very important to achieve the goal of constructing a harmonious and well-to-do society in rural areas. Reinforcing the study on the rural health works especially the health status of rural residents has become one of the main contents of building the rural social insurance system.In rural China, the rank orders of injury in the sequence of disease, death reason and the reason of hospitalization have increased in recent years. Poisoning and trauma has become a familiar disease that occurs frequently and injury has become an important public health problem in rural areas.The epidemiological studies on injury in China were mainly preliminary studies and the scope of those studies was limited. Socio-economic study on injury of rural residents, as well as the study on equity in health outcomes, health service utilization and health finance were never seen, on the other hand, studies on injury of the whole population in rural areas were also seldom seen.Major objectivesThe Main objectives of this study were to analyze the risk factors of injury and calculate the economic burden of injury on the basis of the analysis on the health status and the epidemiology of injury of rural residents; the influencing factors of economic burden were then analyzed. Comparisons were done to determine whether inequity exists in health outcomes, health service utilization and health financing, as well as the degree of inequities. Some indexes were established to calculate the potential economic burden of both injury patients and other patients. Based on the analysis above, some suggestions about health policy and social insurance systems were then put forward.MethodsMuti-stratified random sampling method together with systematic sampling was used to select the rural households, and information was then collected by face-to-face
