Myoma of Uterus; Qi deficiency and Blood stasis ;YiQiHuaYuXiaoZheng Decoction; Clinicial Research
目的:探讨益气化瘀消癥汤治疗气虚血瘀型子宫肌瘤的疗效及作用机理。方法:将60例证属气虚血瘀的子宫肌瘤患者随机分为两组,治疗组30例,口服益气化瘀消癥汤;对照组30例,口服桂枝茯苓胶囊。观察治疗前后临床表现与中医证候改善情况,子宫肌瘤的大小变化情况以及子宫血流动力学、女性内分泌激素、血液流变学和免疫学指标的变化。结果:益气化瘀消癥汤治疗气虚血瘀型子宫肌瘤的总有效率为90%,对照组总有效率为63.33%,两组比较有显著性差异(P<0.05);治疗组对症状的改善除小腹疼痛和头晕目眩方面明显优于对照组,有显著性差异(P<0.05);两组患者治疗后的血液粘滞状态均有明显改善;治疗组患者CD_3、CD_4均升高,CD_8下降,CD_4/CD_8比值升高,NK细胞活性升高,与对照组比较均有显著性差异(P<0.05)。结论:益气化瘀消癥汤能有效缩小子宫肌瘤,明显改善临床症状,改善部分血液流变学指标,调节机体免疫功能,增强抵抗力,而且不影响患者外周血中雌、孕激素水平,无低雌激素血症发生。
Objective To observe the clinical effect and mechanism of YiQiHuaYuXiao Zheng(YQHYXZ) Decoction to treat the myoma of uterus. Methods 60 patients with Myoma of uterus diagnosed Qi deficiency and Blood stasis syndrome were divided randomly into test group (30 cases, treated with YQHYXZ) and control group (30 cases, treated with GuiZhiFuLing capsule).Observe the changes of clinical symptoms and signs, size of the myoma, blood supply of uterus, serum hormones(E_2 , P)levels, blood rheology and immunity. Results The total effective rate of the test group were 90%, while those of the control group were 63.33%.The difference between them was significant (P<0.05) .YQHYXZ is effective to improve the symptom, physical sign and blood rheology and contract the size of myoma. After treatment , YQHYXZ can increase CD3 , CD4 and decrease CD8 in serum with accordingly heightening the ratio of CD4/ CD8.Also the activity of NK cell was increased significantly. The test group is superior to the control one ( P<0.05 ) . Conclusion YQHYXZ can contract the myoma and improve the clinicial symptoms obviously.The effect may be achieved by improving the blood rheology, rectifying the immunity dysfunction. In addition, the side effect such as amenorrhea did not occur , while YQHYXZ did not decrease the level of E_2 and P in serum.
