Medical Secretes of an Official;external treatment;literature
本课题旨在对《外台秘要》中的外治方法进行系统研究，以期对唐代以前的中医早期外治法有一个全面的了解，为现代外治临床和实验提供借鉴和参考。 本论文包括现代中医外治文献综述和《外台秘要》中医外治文献整理研究两大部分。 现代中医外治文献综述部分首先回顾了近二十年对古代中医典籍中中医外治方面所作的整理和探讨研究情况，总结了整理古代外治文献的角度和方法，然后回顾了中医外治机理的研究进展，从外治的理法方药进行了论述，并结合现代经皮给药技术原理分析总结了中医外治的吸收和起效机理。 《外台秘要》中医外治文献整理研究部分，设计表格，逐条检出除灸法、祝由和导引以外的所有外治方论(包括药物外治和非药物外治)，填表后综合整理。 论文先从整体上整理总结了应用外治的数量、病种、方法、剂型和给药部位等。结果共检出外治条文2151条，占全部方论的30.2％，其中用于治疗内病的条文380条，占全部外治的17.7％书中每卷都有或多或少的外治法记载，涉及到广泛的病种，其中一些外病的治疗绝大多数都是外治，由此可见外治法在当时的中医临床治疗中占有重要的地位；并且主要是用于外病治疗。 论文的主体部分是全书外治的具体内容，按照不同的病种分别论述，主要从其所用的外治方法、剂型、给药部位、用药特点和取效标准等方面进行整理，结合不同疾病的病因病机，参考《新修本草》等当时的药物典籍，适当的分析其起效机理和外治规律。内病外治部分，主要介绍了各种不同的疗法、用药部位和取效标准，并举例说明；外病外治多为病变局部用药，主要总结了各疾病的常用药物、剂型和操作。 纵观全书外治文献，《外台秘要》中的外治疗法主要遵从《黄帝内经》中的治疗原则，根据病因病位采取因势利导的方法；寒者热之，热者寒之；重视机体的气血流通，多用温热疗法和温热、芳香、开窍药物，重视外治驱邪和窍道给药；在危重病的急救中大量使用外治；除了治疗疾病，还将外治用于预防疾病和药后调护；所用的外治方药繁简并存，有的组成复杂，制作精细，甚至一方多用，有的则药物较少，属单方验方，制作简单，方便使用，还有一些专病专药；提出一些促进外用药物吸收的方法，如有的外用药须配合手法操作，促进药物到达病位；外科手法器械手术治疗虽然记载较少，但可见其操作的精细。以上各个方面都有我们的临床治疗和实验研究可以借鉴的地方。 书中许多方药既可内服又可外用，部分内服药物须和外用药物配合使用，还有几种治疗方法联合应用，从这可以看出当时医家对于各种治疗方法是一视同仁的，根据疾病的不同需要选择不同的疗法，必要时同时使用多种疗法来达到综合的治疗目的，没有明显地偏废，这一点对于我们现代临床有很大的启示。
This research makes a systematic study of the external treatment of Medical Secretes of an Official (Wai Tai Mi Yao), aiming to have a thorough and complete understanding of external treatment methods before Tang dynasty, so that it can serve as reference to modern clinics and researches.This dissertation composes the following two parts: Review of literature of TCM external treatment and study on external treatment of Medical Secretes of an Official. The first part looks back to the researches in the past twenty years on external treatment of ancient TCM literature, summarizes the methods commonly used for research. Besides, it accounts the progress of mechanism of TCM external treatment both in TCM theory and modern TTS (transdermal therapeutic system) technique.As the main content of this thesis, the study on external treatment of Medical Secretes of an Official is made by collecting an analyzing all the external treatment entries in the book except moxibustion, magic and physical exercise in forms. It explicates the method, medicine pattern, medication position and characteristics according to different kinds of diseases. By referring to herbs classics of that time xin xiu ben cao, analyzing the mechanism and laws of external treatment. As results goes: 30.2% of the total quantity (2151 items) are qualified entries, of which 17.7% (380 items ) are for internal diseases. Altogether, the book covers a large variety of disease, almost every chapter accounts external treatment at different length, and the majority of external diseases use external treatment. In all, it shows that external treatment plays an important role in TCM clinic therapy and that it is mostly used locally for external diseases.To sum up, external treatment in Medical Secretes of an Official applies the principle of Internal classics. The concrete ways are determined by the analysis of pathogeny and location guided by theory of expelling evil from where it attacks. Warm the cold and cool the heat.Regulation of Qi and Blood is attached great attention, thus warm therapy and warm-hot, fragrant and open-orifice herbs is frequently used. External treatment is vital for emergent and serious diseases, to expel evil. Some of its formula are complicated, while some are simple, one formula could be multiple-used, or specific diseases occupy special formulas. Sometimes, more than two kinds of treatment are used to cure one kind of sichness, i.e. it chooses concrete methods according to different needs of various diseases, and shows little bias to any kind. All the above can be of some value as reference.
