牵张成骨(Distraction Osteogenesis,简称DO)技术近年来逐渐在口腔颌面外科领域被广为应用。牵张成骨技术是建立在张力应力法则原理基础上的,但是应力应变影响牵张成骨过程中的成骨作的机理还不十分清楚。虽然临床工作中已经根据前人的经验,设定了牵引前的间歇期,牵引的速率、频率,牵引后的固定期,但是这些参数的变化如何影响牵张区应力应变的变化,进而影响新生骨的构建与重塑,尚不清楚。鉴于以上原因,本研究通过建立兔下颌骨牵张成骨过程中应变测试模型,对牵张成骨过程中的间歇期、牵引速率和固定期内的应变过程进行了测试与分析,试图从生物力学的角度分析牵张成骨的过程,进一步阐明牵张成骨的作用机理。 本研究进行了以下实验。 第一部分:建立兔下颌骨牵张成骨过程中的应变测试动物模型。挑选出合适的手术部位以及包封材料。于一侧下颌骨体部与下颌骨升支结合区骨切开,安装改制的牵张器,间歇4天后开始牵张,0.5nm/次,2次/天,连续5天,于2周后取材,进行大体、X线和组织学观察。结果发现:大部分硅橡胶包封的动物完成了牵张测试过程。动物出现咬合偏斜,新生骨与原下颌骨自然连续。大体标本可见松质骨,X线片表现为局部低
Distraction osteogensis (DO) are becoming wildly accepted in the field of oral and maxillofacial surgery in recent years. It"s based on the principle of strain-stress whose effects on new bone generation during DO is still not known. Although in clinical works, the latency time, distraction rate and frequency, and consolidation time have a common guide, the real effects on bone remolding via the change of strain and stress on distraction area are not well-known. So, in this experiment, we established a rabbit model that could take record of strain during DO. We studied the strain change during latency time, distraction and consolidation time, respectively. To evaluate the effects of distraction osteogenesis, we analyzed the strain trend during distraction from the sight of biomechanics.Part I: This part was to establish an animal model with strain-testing measurement during mandible distraction osteogenesis, including right operative position and encapsulation. After osteotomy of the junction of mandibular body and ramus, setting the customized distracter, and 4days" latency, distraction was managed to lmm/day, twice /day and lmm/day, once/day for 5days, respectively. 2 weeks later, animals were sacrificed for investigation of radiography and histology.
